lxon on "Form plugin which allow multiple rows"

Hi All,

Been searching for form plugin which allow user to enter multiple rows on a form (like phpMyAdmin add record style),
but failed to find relevant plugins so far, guess I'm not alone who want a form plugin which allow multiple rows, hope someone can recommend me.
