I’ve tried a few but looking for a light weight plugin the does the following. The ones I have tried have too many options so I am looking for something basic.
1) User visits home page and sees a form.
2) Fills out the following data. (we log users)
-First Last Name
-Email Address
3) Presses submit.
4) System automatically emails a message to the email provided and sends off info they requested.
